- Findings
- Alleged Victim (AV) relies on the facility for his/her care, has an extensive fall history and Is known to use their call light excessively, for sometimes no apparent reason. AV requires staff assistance with transfers, ambulation, and toileting and is a fall risk due to his/her blood pressure dropping when he/she stands. Between May 7, 2024 and July 22, 2024, AV experienced approximately five falls, and new interventions were not added to AVs care plan consistently after each of the falls. On or about July 25, 2024 AV was found on the floor of their room, yelling for help from staff. AV stated they were walking and suddenly collapsed and were complaining of hip pain. AV was transported to the ER where s/he was admitted to the hospital with a closed right hip fracture and a displaced pelvic fracture. The facility failed to implement interventions and/or appropriately care plan related to AV’s safety and history of falls, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
