- Findings
- Witness 1 (W1) has a history of resident-to-resident altercations, and behavior problems that include hitting, grabbing, and swatting with no specific triggers. Interventions to address W1’s behaviors include staff to sit W1 at a bistro table, as W1 allows, to eat. If staff, see W1 wandering down the hallway they are to redirect W1 to a common area and offer to get W1 an activity. Staff to sit W1 at a table where W1 cannot grab at other residents. If staff, see residents walking close to W1 they are to step in between until the other resident passes. If staff, see W1 approaching other residents they are to redirect W1 to another area and offer a snack. On or about July 28, 2024, AV and W1 are both in the dining room when W1 approaches AV and pulls AV’s hair. W1 then swats towards AV’s chest and head, resulting in a reddened mark to AV’s chest. When W1 and AV were talking, W1 appeared to be in a good mood and then W1's demeanor suddenly changed. W1 required close monitoring. The facility failed to follow W1 service plan, staff were aware that W1 was approaching AV but did not intervene and redirect W1, as is stated in W1’s service plan. which resulted in physical harm to AV when W1 and AV were involved in a resident-to-resident altercation.
