Findings: On or about July 22, 2024, the Alleged Victim (AV) was walking outside and suffered a fall. During this fall, AV was assessed and it was determined AV only suffered a skin abrasion and bruising. On or about July 29, 2024, staff saw additional bruising on AV's arm, from his/her elbow to armpit. AV was sent to the hospital and diagnosed with a fractured arm. AV's behavioral plan states that any time AV suffers a fall, that AV is to be sent out to the hospital. The facility failed to send AV out until a week later, when his/her symptoms progressed. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
