Findings: Alleged Victim (AV) has a known history of falls and has fallen on or about March 12th, 23rd, April 1st, 6th, and July 14, 2024. On or about July 22, 2024, AV experienced another fall while at the facility. The next day, AV was crying and stated AV’s chest was hurting the day after AV fell. As a result of the fall, AV was transported to the hospital and was diagnosed with a fractured sternum and a pleural infusion. The facility failed to provide a safe environment for AV leading to AV sustaining many falls and a fracture, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP25-00108 $500.00 fine assessed
