Findings: According to the documentation, the facility failed to ensure the Alleged Victim (AV) received timely medical treatment for a suspected UTI. On or about July 12, 2024, the AV’s medical provider ordered a urine sample for the AV to test for a UTI. The sample collected by the facility on or about July 12, 2024, was never picked up by the lab. On or about July 23, 2024, the AV returned to the medical provider in pain and unreasonable discomfort with no signs of improvement. The urine sample was found in the fridge at the facility and had been found to not have been sent to the lab, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
