Findings: The Alleged Victim (AV) relies on the facility to assist with transfers and stand by assistance for toilet use. According to an investigation, on or about July 14, 2024, Alleged Perpetrator 2 (AP2) assisted the AV onto the toilet, stated they would not stay to assist the AV off the toilet, and left the AV without their call pendant. Approximately three hours later the AV was found in the bathroom by the next shift. The AV was distressed and no longer wanted to utilize their wheelchair to use the bathroom. AP2’s actions are a violation of resident rights, considered neglect, and constitutes abuse. The facility did not keep the AV free from neglect, which is a violation of Oregon Administrative rules.
