Findings: According to the documentation, the facility failed to provide a safe environment for the Alleged Victim (AV) by not ensuring the door locked appropriately. On or about July 31, 2024, at approximately 4:00 pm, the AV was able to open a secured door which exited the facility before it locked. It was known by the facility there is a delay when the door shuts and when the electronic lock engages. The AV was located by local law enforcement and brought back to the facility. The facility failed to provide a safe environment which placed the AV at risk of harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
