Findings: Both the Alleged Victim (AV) and Witness 1 (W1) rely on the facility for care and safety. W1 has a history of aggressive behaviors resulting in resident-to-resident altercations and is care planned to be separated from the AV due to previous physical altercations. On or about July 31, 2024, W1 pushed AV out of their wheelchair in the common area of the facility, resulting in AV receiving a forehead abrasion and a finger laceration that required stitches. The facility failed to implement interventions to address W1 aggressive behaviors and provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
