Findings: According to the documentation, the facility failed to provide a safe environment to ensure the Alleged Victim (AV) could not leave the secured facility unsupervised. On or about July 31, 2024, the AV was able to use approximately three doors which were supposed to be locked to exit the facility. The oncoming night shift staff saw the AV outside the secured facility and was able to redirect the resident back into the facility. The failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
