Findings: On or about August 3, 2024, the Alleged Victim (AV) was found outside the facility courtyard. The courtyard gate had been left open by the landscaping crew and AV was able to exit the gate outside of the locked facility. AV was found by staff and returned to the facility. AV was found to have scratches on his/her foot and arm. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
