Findings: The Alleged Victim (AV) and Witness 4 (W4) both have a history of behaviors and physical altercations with each other. According to an investigation, on or about August 7, 2024, AV and W4 were left alone in the common area of the facility. When staff arrived, they found W4 standing over AV holding onto their arms and bending AV's fingers backwards. AV complained of head and leg pain and was sent to the hospital and diagnosed with a brain bleed, a fractured hip, and a fractured pelvis. The facility failed to provide services to keep AV safe, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP25-00254 $2500.00 fine assessed
