Findings: The Alleged Victim (AV) relies on the facility for mobility and transfer needs. According to an investigation, staff incorrectly re-positioned the AV while in bed, causing injury and pain to their left shoulder. The facility failed to appropriately care plan and instruct staff on the proper procedure to assist the AV with re-positioning, which is a violation of resident’s rights, is neglect of care and constitutes abuse.
