Findings: Alleged Victim (AV) relies on the facility for his/her care. AV is care planned to have direct supervision by staff when in the dining room, and that the staff member that starts a task with AV needs to stay with AV. On or about August 10, 2024, the Alleged Perpetrator #2(AP2) brought AV to the dining room and then went on break, leaving AV unattended. AV fell out of his/her wheelchair resulting in a five-inch scrape down their back. AP2 failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to ensure the care plan was being followed, which is a violation of Oregon Administrative Rules.
