Findings: The Alleged Victim (AV) has a history of falls and having ticks and/or spasms. AV was assigned one to one care, to be performed by Alleged Perpetrator #2 (AP2). On or about August 9, 2024, AV was walking down the hall when AV had a spasm and fell. AV landed on his/her wrist and was taken to the hospital and diagnosed with a fractured wrist. AP2 failed to ensure AV's safety while ambulating, as AP2 was not close enough to AV to protect AV from falling. AP2's actions are a violation of resident rights, are considered neglect of care and constitute abuse. The facility's failure is a violation of Oregon Administrative Rules.
