Findings: On or about August 13, 2024, the Alleged Victim (AV) was found on his/her bathroom floor after an unwitnessed fall, resulting in a scalp laceration that required hospitalization and staples. Based on facility documentation and interviews, AV had a documented history of falls, including 11 falls between June 19, 2024, and August 9, 2024, two of which resulted in head injuries. The facility's lack of appropriate oversight and care planning regarding AV's known fall risk, caused AV continued pain and unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
