Findings: Alleged Victim (AV) is care planned to have a common behavior of taking food or drinks from residents. Staff are to monitor AV closely as AV often attempts to take other residents’ food off their table during mealtimes. On or about August 18, 2024, AV was wandering in the dining room and reached onto Witness 1’s (W1’s) plate to grab food. W1 grabbed AV’s hand and dug W1’s nails into AV. As a result, AV had a skin tear and bruising on his/her palm of their right hand. The facility failed to assure the care plan was followed by allowing AV to reach into another resident’s plate and causing an altercation, which is a violation of resident rights, is neglect of care and constitutes abuse.
