Findings: According to the documentation, the Alleged Perpetrator 2 (AP2) failed to follow the service plan to secure the Alleged Victim (AV) into their wheelchair. On or about August 14, 2024, the AP2 failed to secure the AV into a seatbelt when they were placed in the back of the bus. Due to the seatbelt not being engaged, the result was the AV falling out of the wheelchair sustaining multiple injuries including bruising and head laceration, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to ensure the service plan was being followed which is a violation of Oregon Administrative Rules.
