Findings: The Alleged Victim (AV) had a history of falls, an unsteady gait, and was care planned for monitoring of a decline and increased weakness. According to an investigation, on or about August 15, 2024, AV experienced an unwitnessed fall in their room, resulting in pain and the inability to bear weight on the left side. The facility failed to provide sufficient interventions to address the AV’s decline and increased weakness, which is a violation of resident rights, is neglect of care, and constitutes abuse.
