Findings: On or about August 19, 2024, the Alleged Victim (AV) was found with feces on his/her person. AV was using napkins from the dining room to try to clean him/herself up. Staff were alerted to AV's need for assistance, however, no staff assisted AV with clean up. AV's room smelled of feces and there were feces on his/herself, the wall near the toilet and on napkins hanging in AV's bathroom. The facility's failure to ensure a clean, safe, environment places AV at risk for harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
