Findings: Alleged Victim (AV) relies on the facility for care and for a safe environment. On or about August 19, 2024, the Alleged Perpetrator 2 (AP2) grabbed AV under their arm and strongly guided AV down the hall against AV's will. AP2’s actions are a violation of resident rights, are considered neglect of care and constitute wrongful restraint/physical abuse. The facility failed to ensure AV was in a safe environment and protected from wrongful restraint/physical abuse which is a violation of Oregon Administrative Rules.
