Findings: The Alleged Victim (AV) resides at the facility and relies on facility staff for cares. On August 12, 2024, AV was attempting to go into another residents room when Alleged Perpetrator #2 (AP2) grabbed AV by the arm and cursed at AV. AV was fearful the rest of the night. On or about August 17, 2024, AV was noted to have bruising on his/her upper arm which looked like finger marks. On or about August 18, 2024, AP2 grabbed AV by the wrist and forcefully led AV to the TV room, trying to make him/her sit down, while cursing at AV. Staff intervened and told AP2 that he/she could not treat AV this way. AP2's actions are a violation of resident rights, are considered neglect of care and constitute physical and verbal abuse. The facility's failure to protect AV from physical and verbal abuse is a violation of Oregon Administrative Rules.
