Findings: The Alleged Victim (AV) relies on the facility for his/her care. AV is known to have a history of urinary tract infections (UTI’s) and bladder incontinence. AV was care planned for total assistance with toileting, showering, dressing, and housekeeping, occasional assist with ambulation, and standby assist with transfers. According to an investigation, on or about August 25, 2024, at approximately 8:00am, AV was found in another resident’s room in sitting on his/her urine for an extended period. EMS was called and found AV to be hot to the touch, smelling strongly of urine, and was unable to hold a conversation. AV was taken to the hospital for extreme weakness, confusion and diagnosed with a UTI and blood in his/her urine at the hospital. The facility failed to provide appropriate services, related to care planning and following AV’s care plan, which is a violation of resident rights, is neglect of care which constitutes abuse.
