Findings: The Alleged Victim (AV) has exit seeking behaviors. According to an investigation, on or about August 26, 2024, AV eloped from the facility and was found between 4:30pm – 5:00pm in the front parking lot area, which is adjacent to a busy intersection, posing a serious risk of harm to AV. It was discovered facility staff conducted a safety exercise between 3:08pm – 3:30pm, leaving the doors to the security portion of the facility unlocked. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care which constitutes abuse.
