Findings: On or about August 30, 2024, it was determined by staff that the Alleged Victim's (AV) medication amount was off. A facility investigation determined that the medication administration system and the narcotic log did not match. According to the narcotic log, AV missed on dose of his/her medication. AV did not have any adverse reaction to missing the medication. The facility's failure to ensure a safe medication administration system is a violation of Oregon Administrative Rules.
