Findings: According to the documentation, the facility failed to follow the care plan for Witness 1’s (W1) known behavior of sexual behavior. On or about September 02, 2024, when trying to locate W1 and the Alleged Victim (AV), they were both found in W1’s room. The AV was found on W1’s bed with not pants on and W1 was fully nude in the room. The AV was immediately dressed and redirected out of the room. There were service plan updates to separate W1 and the AV if they were seen together. Earlier in the day, on or about September 02, 2024, AV and W1 were seen holding hands and there was no documented evidence the facility redirected the residents away from each other which is a violation of resident rights, is considered neglect of care and constitutes abuse.
