Findings: The facility failed to provide appropriate services according to the Alleged Victim’s (AV) care needs and wound care. Based on facility documentation and interviews, the facility failed to change AV's wound dressing after AV was released from the hospital. Based on facility documentation and interviews, AV wore the same wound dressing from August 22, 2024, through September 4, 2024. The facility failed to clean or dress AV's forearm wound which caused AV’s wound to become infected, causing AV repeated unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
