Findings: According to the documentation, the facility failed to properly care plan appropriate interventions for Witness 1’s (W1) known behaviors which affect other residents. On or about September 04, 2024, W1 pushed the Alleged Victim (AV) down as they were walking by their room, causing the AV to fall and hit their head. W1 has known behaviors which result in resident-to-resident altercations. The facility’s failure to implement appropriate interventions for W1 behaviors to prevent resident to resident altercations is a violation of resident rights, is considered neglect of care and constitutes abuse.
