- Findings
- The facility failed to implement interventions and appropriately monitor the Alleged Victim and Witness 1 according to their known behaviors and prior altercations. The failure resulted in the residents experiencing an unwitnessed physical altercation causing unreasonable discomfort to AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
