Findings: On or about September 6, 2024, Alleged Perpetrator #2 (AP2) and Alleged Perpetrator #3 (AP3) were transferring the Alleged Victim (AV) using a hoyer lift. AV slid out of the sling and hit his/her head, causing a laceration to his/her head. The sling that was used was too large for AV, causing him/her to slip. While the facility had trained AP2 and AP3 on using the hoyer lift, the training did not include how to select the correct size to fit each resident. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
