Findings: The Alleged Victim (AV) relies on the facility for care. AV is care planned for assistance with showers two times per week and it is documented that AV prides themselves on their appearance. According to an investigation, AV was described as having a disheveled appearance and sometimes greasy hair. Documentation reveals that during the month of September, 2024, AV was only showered a total of four times, causing a loss of personal dignity. The facility’s failure to provide basic care and services for AV resulted in a loss of personal dignity and is a violation of resident’s rights, are considered neglect of care and constitutes abuse.
