Findings: The Alleged Victim (AV) relies on the facility to provide a safe environment. According to an investigation, the facility was aware that when AV moved in, AV was a smoker, and the facility is a non-smoking facility. AV attempted to elope from the facility multiple times between August 16 and August 28, 2024, and successfully eloped twice. The facility failed to provide AV with a safe environment, which is a violation of resident rights, is neglect of care which constitutes abuse.
