Findings: Alleged Victim (AV) relies on facility staff to administer medications. According to an investigation, on or about, September 8, 2024, Alleged Perpetrator 2 (AP2) administered another resident's medication to AV, resulting in AV being transferred to the hospital. AP2's actions exposed AV to a risk of potential serious harm, are considered neglect and constitutes abuse. Based on the investigation, it was determined that the facility failed to ensure AV's medication was administered as ordered. The facility exposed AV to potential serious harm, which is a violation of resident rights, is neglect of care which constitutes abuse.
