- According to an investigation, between August 23, 2024, and September 3, 2024, an unknown Alleged Perpetrator 2 (AP2) made unauthorized charges using AV’s credit card. AP2’s actions are considered theft and constitutes financial exploitation. The facility failed to protect AV from financial exploitation, which is a violation of Oregon Administrative Rules.
