Findings: Witness #1 (W1) is known to have behaviors and altercations. W1 has been involved in approximately 8 altercations in 2024. On or about September 13, 2024, W1 was in the hallway and was upset. The Alleged Victim (AV) walked past W1 and W1 pushed AV to the ground unprovoked by AV. AV was not injured, however, AV was placed at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
