Findings: The Alleged Victim (AV) is a resident of the assisted living facility with his/her spouse. On or about September 18, 2024, AV was taken by staff to the memory care side of the building due to AV's spouse being out of the facility for a medical event. While AV was in the the memory care, AV had been sitting in the dining room. AV got up from the dining room and wandered down the hallway and fell. Emergency services were contacted and AV was transported the hospital and diagnosed with a fractured hip. The facility's failure to ensure a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
