Findings: Witness #1 (W1) is known to be anxious and agitated at times. Between September 1, 2024 and September 17, 2024, W1 experienced 8 episodes of anxiousness and confusion. On or about September 22, 2024, W1 pushed the Alleged Victim (AV) during an altercation, causing AV to fall and hit his/her head on the wall. AV was transported to the hospital where he/she received 3 staples to close the wound on his/her head. The facility's failure to ensure a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP25-00009 $1125.00 fine assessed
