Findings: The Alleged Victim (AV) relies on the facility for a safe environment. Witness 1 (W1) has a known history of aggressive behaviors. The facility failed to appropriately care plan and/or implement specific interventions in place to monitor the AV and W1. The facility failed to ensure W1 was administered their scheduled behavior medication. According to an investigation, on or about, September 24, 2024, W1 threw the AV against a wall, resulting in injury and unreasonable discomfort. The AV was sent to the hospital with a contusion to the head. The facility failed to provide a safe environment, which is a violation of resident’s rights, is neglect of care, and constitutes abuse.
