Findings: Alleged Victim (AV) has a known history of falls. On or about September 27, 2024, at 6:20am, staff found AV on the floor with a skin tear on his/her knee. On or about September 28, staff went to AV’s room and found AV on the floor experiencing chest pain. There are no interventions in place to alert staff when AV attempts to stand, transfer, or walk independently in his/her room, and AV is unable to utilize the call light effectively to call for help. The facility failed to properly plan care around AV’s known history of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
