Findings: The Alleged Victim (AV) relies on the facility for care. AV is a known fall risk with a history of falls. AV is care planned for stand by assist while ambulating. According to an investigation, on or about September 26, 2024, AV returned to the facility from a two-day hospital stay by paramedics who escorted AV into the facility and their room. Approximately ten minutes after returning, AV suffered a fall with injury in their room, resulting in a head laceration that required stitches. The facility failed to provide a safe environment, which is a violation of resident rights and is neglect of care which constitutes abuse.
