Findings: The Alleged Victim (AV) and Witness #1 (W1) have a history of altercations with each other. Staff are to monitor AV and W1 for agitation and redirect when necessary. On or about September 28, 2024, AV and W1 were in the dining area when W1 slapped AV in the face. No staff were present during the altercation, but heard AV screaming and arrived to separate the two residents. The facility's failure to ensure resident safety is a violation of resident rights, is considered neglect of care and constitutes abuse.
