Findings: The Alleged Victim (AV) relies on the facility for his/her care. According to an investigation, during a brief change on or about October 8, 2024, Alleged Perpetrator 3 (AP3), did not follow AV’s care plan, wrongfully restrained AV, and forcefully changed AV’s brief, resulting in a right shoulder injury and a left wrist injury. AP3’s actions constitute neglect, wrongful restraint and physical abuse. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP25-01008 $500.00 fine assessed
