Findings: On or about October 09, to October 10, 2024 AV was found soaked in urine. AV care plan indicates staff are to check AV no less than four times a shift, and reposition AV each time. During checks AV is to have brief checked for urine and stool, if AV is soiled AV is to be cleaned and changed into a new brief. Alleged Perpetrator 2 (AP2) was responsible for AV's section on or about October 09, to October 10, 2024. The facility failed to provide oversight, and make sure Alleged Victim's care plan was being followed, which violates Oregon Administrative Rules. The Alleged Perpetrator failed to follow AV care plan. AP2’s actions are a violation of resident rights, considered neglect and constitutes abuse.
