Findings: The Alleged Victim (AV) is full assist with toileting and was care planned to be toileted beginning, middle and end of each shift. On or about October 9, 2024 and October 10, 2024, during night shift, AV was only checked on one time, at 2:09 AM, according to documentation. Alleged Perpetrator #2 (AP2) was working this shift and was responsible for the section that the AV was in. On the morning of October 10, 2024, AV was found soaked in urine head to toe in his/her room. AP2 had not changed AV the entire shift, leaving AV in unreasonable discomfort. AP2's actions are a violation of resident rights, are considered neglect of care and constitute abuse.
