Findings: The Alleged Victim (AV) is a one person transfer assist with transferring and needs daily assistance, reminders, cues and standby for toileting and was care planned to be checked on at least 4 times per shift. On or about October 9, 2024 and October 10, 2024, during night shift, AV was not checked on during this shift at all. Alleged Perpetrator #2 (AP2) was working this shift and was responsible for the section that the AV was in. On the morning of October 10, 2024, AV was found soaked in urine head to toe in his/her room. AP2 had not changed AV the entire shift, leaving AV in unreasonable discomfort. AP2's actions are a violation of resident rights, are considered neglect of care and constitute abuse. The facility's failure is a violation of Oregon Administrative Rules.
