Findings: The Alleged Victim (AV) relies on the facility for his/her care. AV has a history of refusing care and known to be incontinent and remain his his/her soiled clothing/briefs, resulting in AV being diagnosed with a UTI on multiple occasions and loss of personal dignity. The facility failed to provide appropriate services for AV’s needs, including care planning and lack of instruction, which is a violation of resident rights, is neglect of care which constitutes abuse.
