- Findings
- The Alleged Victim (AV) relies on the facility for his/her care. According to an investigation, AV experienced approximately four unwitnessed falls between July 2, 2024, and September 28, 2024, resulting in pain, contusion to AV’s left hip and unreasonable discomfort. The facility failed to appropriate care plan and provide services according to the Alleged Victim (AV)'s needs, relating to falls and lack of appropriate interventions, which is considered neglect of care and constitutes abuse.
