Findings: The Alleged Victim (AV) relies on the facility for his/her care. The AV is care planned for assistance with transferring and toileting and to be checked on eight times per shift, including night shift. According to an investigation, on or about October 11, 2024, AV used the call button and when staff took too long to respond, AV transferred themselves and attempted to self-toilet which resulted in AV urinating in his/her pants. AV experienced unreasonable discomfort and a lack of personal dignity. It was also discovered that AV’s care plan was not being followed regarding safety checks. The facility failed to provide appropriate services according to resident needs, as it relates to care plan not being followed, and call light not being answered in a timely manner, which is a violation of resident rights, is neglect of care which constitutes abuse.
