Findings: The Alleged Victim (AV) relies on the facility for his/her care. The AV is care planned for assistance with transferring and toileting and to be checked on eight times per shift, including night shift. According to an investigation, on or about September 28, 2024, AV transferred themselves out of his/her powered wheelchair, resulting in AV hitting his/her head. The facility failed to provide appropriate services according to Alleged Victim’s needs, relating to care planning and lack of appropriate interventions and/or call light not being answered in a timely manner, which is a violation of resident rights, is neglect of care which constitutes abuse.
