Findings: The Alleged Victim (AV) relies on the facility for his/her care. According to an investigation, AV was left sitting in urinated clothing for long periods of time, resulting in AV getting a rash in his/her groin area and causing unreasonable discomfort and lack of dignity. Witnesses indicated that facility staff were not aware that AV had a rash on his/her groin area. The facility failed to provide appropriate services for AV’s needs, including care planning and lack of instruction, which is a violation of resident rights, is neglect of care which constitutes abuse.
