Findings: The Alleged Victim (AV) relies on the facility for his/her care. The Alleged Victim (AV) was care planned for assistance with transferring, reposition, toileting, and total assistance for dressing. AV had a pressure sore that that fluctuated in severity. According to an investigation, AV was found soaked in urine by multiple staff members on many separate occasions due to lack of staffing, resulting in unreasonable discomfort, lack of dignity and placing AV at risk of serious harm. The facility failed to follow AV’s care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
